What are ‘Swing Bed’ Services?

To better serve the needs of our rural community, Cordell Memorial Hospital offers a Swing Bed program. The Cordell Memorial Hospital Swing Bed program gives you the opportunity to heal in a caring environment close to home!

Not everyone is ready or has the ability to return to everyday living after a hospital stay. Our Swing Bed program allows you to take the extra time needed to heal and make a smooth transaction between acute care and returning home. Benefits include being close to home and your loved ones, skilled nursing care, on site physical therapy.

Swing Bed refers to the practice of “swinging” patients from one level of care-acute care that requires hospitalization-to the next level-continued skilled nursing and rehabilitation services as you continue your recovery. The Cordell Memorial Hospital Swing Bed Program is a Medicare-certified skilled nursing program that provides patients with short-term rehabilitative therapy, which includes skilled nursing care and/or rehabilitation services. The Swing Bed program is a step-down level of care for patients who are not ready to return home after a hospital stay, need more time to strengthen and heal before they can step back into the routine of independent living.

For a patient to qualify for the Swing Bed program, they must have an ongoing diagnosis and requires skilled care as defined by Medicare. Who needs Swing Bed Care?

• Patients needing continued wound care

• Patients who need IV antibiotic therapy for infections

• Orthopedic rehabilitation after hip/knee surgery

• Stroke rehabilitation - non acute

• Management of a chronic illness after an acute episode that required hospitalization. Who qualifies for Swing Bed?

Swing Bed generally qualifies Medicare coverage with the following specific conditions.

• Your Swing Bed care must be related to an acute hospital stay

• You must need skilled nursing or skilled rehabilitation services on a daily basis and must be complex enough that services must be performed by or under the supervision of professional or technical personnel.

• You must have had a three-day qualifying stay in an acute care setting in the past 30 days.

The length of stay is determined by the physician and therapist based on how the patient meets goals and progresses during the stay. The average length of stay is 10-12 days. Medicare covers up to 20 days at 100% coverage with no out-of-pocket expense to the patient, provided that the patient meets the requirements to need this skilled care.

Our goal is that each patient will achieve their highest level of function and will be able to return home. If the patient is unable to return home on discharge, we will assist the patient and family in finding the right type of care to meet their needs

We encourage patients and their families to request Swing Bed services at Cordell Memorial Hospital to help the patients remain closer to home and their family. Simply let your discharge nurse know about your preference.
